Implementation of the national health project in urologic cancer patients at the regional level: The first results

Objective - to summarize the first results of implementing the national heath project for urologic cancer patients in case of the health care system of the Kemerovo Region. Material and methods. The database of state and specialized statistics, the statistics of the uniform information space of public health in the sections of oncology and uronephrology over 2011 -2014 and the planning of health care system development in implementing the state program «Health Care Development in Kuzbass in 2014 - 2017». Results. The state program «Health Care Development in Kuzbass in 2014 -2017» was found to be launched when healthcare staffing was rather poor; the urologic staffing in the region's out- and inpatient settings was only 67.8%. The 22.3% reduction in the specialized urologic bed space could not be compensated for at the out- and inpatient level. Conclusion. The implementation of the state program «Health Care Development in Kuzbass in 2014 -2017» at the first stage (2013 -2014) failed to result in a significant increase in the detection rate of urologic cancer. The authors consider that this is associated with staff scarcity (for example, the urologic staffing in the region's out- and inpatient settings was only 67.8%; the financing of primary outpatient care was poor). And the main point is that an emphasis could not be shifted from expensive and resource-intensive inpatient care to the economically sound and accessible outpatient care. The predominance (52.1%) of operated Stages III and IV urologic cancers suggests that the patients have sought medical advice late and urologic cancer has been misdiagnosed in the regional healthcare facilities.
