Surgical complications of prepectoral breast reconstruction with implants

Reconstructive and plastic surgeries with immediate prepectoral implant placement in patients with breast cancer (BC) are becoming increasingly relevant. Excellent aesthetic outcomes and high quality of life are the primary incentives for performing such procedures. The experience of this intervention over more than 10 years in patients with BC, including those after systemic combined treatment, allows for the evaluation of its outcomes and the potential for complications. This article presents the results of the follow-up in the Department of Oncology and Reconstructive Plastic Surgery of the Breast in patients after immediate breast reconstruction with prepectoral placement of textured (45%) and polyurethane-caoted (47.5%) implants, as well as tissue expanders (7.5%). The follow-up period was 65 months. The study assessed the incidence of complications such as seroma, hematoma, red breast syndrome, rippling, wound edge dehiscence, implant loss, and grade III–IV capsular contracture. Additionally, clinical examples of certain complications are provided. Analysis of literature and personal experience suggest that careful patient selection and improvement of surgical techniques contribute to the further dissemination of this method and the minimization of potential complication risks. © 2024, Meditsina-Inform LLC.
