The prevalence of aortic stenosis with concomitant abdominal aortic aneurysm directly related to age and usually occurs in elderly patients. Both conditions lead to adverse outcomes if not adequately managed. Patients of this age often have comorbid conditions and a history of previous surgical interventions. This article represents a clinical case of successful simultaneous transcatheter aortic valve implantation (TAVI) and endovascular abdominal aortic aneurysm repair in a hybrid operating room to a high-risk patient with severe aortic stenosis and concomitant aneurysm of the infrarenal aorta. The intervention was complicated by displacement of the stent graft followed by covering of the renal artery. We failed to perform revascularization. Besides the common femoral artery plasty was done due to discovered dissection. Acute kidney injury occurred in postoperative period and was successfully treated conservatively. Despite on the good short-term result of the intervention, the occurred complication demonstrated that careful selection of patients, attention to peculiarities of the patient’s anatomy and a multidisciplinary approach at all stages of treatment are essential. © 2023 Authors.
