Risk factors affecting the development of complications in patients with giant incisional hernia [Факторы риска, влияющие на развитие осложнений у пациентов с гигантскими послеоперационными вентральными грыжами]

Background. The formation of incisional hernias remains a fairly frequent complication after laparotomy, and successful treatment is assessed not only by the absence of recurrence, but also by the absence of postoperative complications. Aim. To evaluate the impact of risk factors on the development of complications in patients with incisional hernias after component separation. Material and methods. The analysis of surgical treatment of 91 patients with giant incisional hernias was carried out. Patients were divided into 2 groups. Patients in the first group underwent posterior component separation, patients in the second group underwent anterior component separation. The analysis of the influence of risk factors (age, obesity, diabetes, oncology, long history of smoking) on the development of complications in the postoperative period was carried out. Results. Univariate analysis did not show the influence of individual risk factors on the development of local and general complications in patients in both groups. Multivariate analysis revealed the relationship of diabetes and obesity, as well as the combination of diabetes, obesity, smoking and their impact on the development of local complications in patients after anterior component separation. Conclusion. Comprehensive assessment of preoperative health and risk factors can significantly affect postoperative outcomes. © 2020 GEOTAR Media. All rights reserved.
