Ovarian reserve and innovative salpingectomy technique in patients with bilateral hydrosalpinx before initiating ART

Data on how bilateral salpingectomy affects ovarian reserve is available in the literature. Given the recommendations of reproductive specialists regarding the need to remove the fallopian tubes before entering ART programs, it is important to improve salpingectomy techniques in order to minimize trauma to the functional capacity of the ovaries. Objective. To study the efficacy of the original salpingectomy technique and its impact on the reproductive potential of women with bilateral hydrosalpinx before initiating ART. Patients and methods. A prospective multicenter cohort study was conducted. The results of examination and treatment of 100 infertile patients with bilateral hydrosalpinx aged 28 to 35 years were studied. Two patient groups were identified. In group (n=50), the procedure was performed using the original technique with preoperative 3D modeling and intraoperative surgical navigation. In group 2 (n=50), salpingectomy was performed using a standard technique. Results. The use of the original salpingectomy technique did not affect any of the baseline parameters of ovarian reserve (p > 0.05). In the group of patients who underwent salpingectomy without prior identification of vascular zones, a comparative analysis of ovarian reserve parameters before and after surgery revealed its pronounced suppression: AMH – 2.27 ± 0.3 and 1.95 ± 0.5 ng/mL, AFC – 7.0 ± 0.6 and 5.1 ± 0.9, respectively. Conclusion. The original technique with preoperative modeling and surgical navigation in patients with bilateral hydrosalpinx allows the source of chronic inflammation to be removed without disrupting the blood supply to the ovaries and their reproductive potential. © 2025, Dynasty Publishing House.
