A study of the lesions of the major bile ducts in benign biliary tract pathology using various preoperative and special intraoperative methods of examination permitted the authors to diagnose in several cases segmental and combined obstructive lesions of the hepatocholedochus and papilla of Vater. The latter were due to gallstones, cicatricial stenoses or compressing pancreatitis in varying combinations. The authors suggest a double internal drainage procedure of the hepatocholedochus for the elimination of such lesions that is accomplished in the form of a transduodenal papillosphincterotomy and supraduodenal choledochoduodenostomy. The latter intervention may be performed in one stage, or consecutively in the course of a reoperation following an unsuccessful transduodenal papillosphincterotomy or supraduodenal choledochoduodenostomy. The indications for such interventions were determined, and 15 personal observations are presented.
