Приведены результаты исследования эффективности психокоррекции и премедикации для проведения стоматологических манипуляций у больных шизофренией. Предложено использование специальной шкалы (ШКС - шкала клиническая стоматологическая) для оценки эффективности и выбора метода подготовки больного шизофренией к проведению стоматологического обследования.
The article deals with the results of psychocorrection and medicamentous preparation of patients with schizophrenia for stomatologic treatment. The authors proposed different methods of clinical assessment of effects of premedication. Special scale for express diagnosis of anxiety level was elaborated. Dynamic clinical-stomatological, clinical-psychopathological, clinical-follow-up, experimental-psychological examinations of 60 stomatologic patients with schizophrenia was carried out during 2006-2007 years in psychiatric hospital of Moscow. The mean age of observed patients were 30 years. High level of stomatologic pathology and even loss of teeth in young people with psychic patology was revealed. It was due to their postponed attendance and even denial to visit dentists, lack of care for teeth, abnormal reactions to stomatologic examination. 5 general types of abnormal psychic reaction to stomatologic examination were described. Deterioration of mental state caused by reaction to primary stomatologic examination influenced efficiency of treatment and demanded preliminary diagnostic investigation and treatment. Preventive measures and early revealing of mental exacerbation in stomatologic patients with schizophrenia guarantees maintenance of medical assignments, considerably improves compliance of the patient with the doctor as well as ameliorates current condition and outcomes of caries in patients admitted to mental hospitals. Use of the questionnaires and application of psychocorrectional methods as well as correct selection of premedication drugs are purposeful for early revealing of both psychiatric and stomatologic problems considerably improves cooperation between patients and psychologists or the attending physicians and accordingly improve the prognosis of disease.
