Пренатальное выявление неразделившихся близнецов (торакоомфалоисхиопагов) в I триместре беременности

Представлен случай пренатального выявления торакоомфалоисхиопагов при проведении скринингового ультразвукового исследования в I триместре (13+1 нед) у 36-летней повторнобеременной пациентки. Беременность прервана в сроке 14+1 нед. Это 28-е сообщение в отечественной литературе о пренатальном выявлении неразделившихся близнецов начиная с 1989 г. Своевременная пренатальная диагностика позволяет в одних случаях (при бесперспективном прогнозе) прерывать беременность на ранних этапах, а в других - ее пролонгировать, планируя своевременное постнатальное хирургическое разделение близнецов.

Prenatal detection of conjoined twins (thoracoomphaloischiopagus) in the I trimester of gestation

Conjoined twins is a rare and sporadic event which generally occurs after the 13th day after fertilization and results from incomplete separation of the monozygotic embryonic disk. The incidence of this situation is reported to be in the range of 1 in 22 000 to 200 000 births. We present a case of a 36-year-old multigravida who visited antenatal clinic for the first screening sonography at 13+1 weeks of gestation. Conjoined twins (thoracoomphaloischiopagus) were detected. Pregnancy was terminated at 14+1 weeks. Advances in prenatal diagnosis allow detailed, accurate evaluations of conjoined twins. Detection of conjoined twins is not difficult even with ultrasound units of middle class. Careful prenatal studies may uncover cases in which emergent separation at birth is lifesaving.
