Objective. To study the effectiveness of three-dimensional (3D) modeling and intraoperative navigation in the surgical treatment of uterine fibroids (UF). Patients and methods. A total of 100 patients with UF who underwent laparoscopic myomectomy were examined. Patients were divided into two groups. In the study group of 50 patients, a 3D model of the uterus was constructed with registration of the topography of fibroid nodules and their blood supply zones during the preoperative examination. The comparison group consisted of 50 patients who underwent standard preoperative examination. Results. In the study group, a low degree of surgical traumaticity was observed in 76% of patients and a moderate degree – in 24%; in the comparison group – in 16% and 84%, respectively (p < 0.05). Preoperative modeling of surgical intervention also helped to reduce intraoperative blood loss. Minimal intraoperative blood loss (50 mL) was detected in 42 (84%) patients in the study group, moderate (50–100 mL) – in 8 (16%) patients, in the comparison group – in 6 (12%) and 29 (58%) patients, respectively. Conclusion. Preoperative 3D modeling makes it possible to plan surgical interventions considering the individual features of the topography of fibroid nodules, vascular zones in fibroid nodules and myometrium.
