Secondary gluteoplasty: Options for surgical correction

Objective. To analyze the incidence of complications and need for redo surgery after buttock augmentation. Material and methods. The authors report redo surgeries in 26 patients with complications after primary buttock augmentation with silicone implants. Period after primary gluteoplasty ranged from 6 months to 9 years (mean 3.3 years). Indications for surgery were implant contour-ing, buttock ptosis and implant rupture. Results. Implant replacement was performed in 15 patients including preservation of implantation layer in 10 cases and change of this layer in 5 cases. The implant was removed in 11 patients. Six of these patients required myopexy for closure of residual cavity. This procedure was supplemented by upper lifting in 3 cases and lower lifting in 1 case. All patients noted certain improvement after surgery. Conclusion. Integumentary tissue thickness should be considered to choose the method of secondary correction. Preservation of the existing implant cavity is possible if integumentary tissue thickness is over 25 mm. On the other hand, a deeper cavity is required if thickness of the un-derlying layer of gluteus maximus muscle is over 18 mm. Removal of implants is indicated in other cases.
